Our primary care doctors offer a full range of family practice medicine for children, adults and seniors. Regardless of your age, our physicians are dedicated to giving you or a loved one the patient-centered care everyone deserves. We also have full laboratory services in our practice for your convenience.

Before Your First Appointment

Bring the Following to Your First Appointment:

  • Photo ID
  • Insurance Card
  • Current list of medications (or bring your medications with you)
  • Preferred pharmacy name, address and phone number
  • Forms:  Download our new patient forms from the forms page, print and fill out, and bring to your first appointment.

Insurance Verification

Determining which doctors are in-network and which are not can be a hassle. But, not for you at OneSource! Give us a call and we can handle that for you with your insurance company. Your insurance verification will go more smoothly if we have a copy of your insurance card prior to your first visit. You may email a copy (front and back) of your insurance card to us prior to your first appointment. You may also fax securely or mail. Albany Office Fax Number: 229-439-1951. Bainbridge Office Fax Number: 229-246-2041. If you do not bring your insurance card to your visit and we cannot verify your insurance, you will be considered a self-pay patient until your insurance can be verified. We will charge your credit card at the time of visit. The charge can be reversed if we receive your insurance information within two (2) business days. We appreciate your understanding.
